DPC is a membership-based healthcare model designed to give you unlimited, easy access to your provider without the  hassle of traditional insurance. Instead of co-pays and deductibles,  DPC uses a flat, monthly fee that covers everything from routine visits  to preventive care.

  • Initial one time enrollment fee of $75.00 per person. 
  • A minimum 3 months of membership is required to sign up. 
  • Initial sign-up fee will vary for family and business plans. 
  • If you once had a membership and wish to re-enroll a $75 rejoining fee will be assessed, in addition to the standard $75 enrollment fee.
