Client Forms

Prior to receiving your first SecondWind IV & Wellness service, you will need to fill out the required forms. These forms are then reviewed by our Nurse Practitioner to determine eligibility. We recommend filling these out prior to visiting for services to save time.

• Visit our booking site at: www.vagaro.com/secondwindiv
• Click on the red 'Book Now' button.
• Under 'Select Service' choose 'Complete Required Forms'. If you do not have an account with Vagaro, you will need to create one to complete this step.
• Choose any time and date. This does not commit you to any service or specific time, but will allow you to fill out the necessary paperwork.
• You will then receive notice when you've been approved for your selected services.
